OUR SERVICES: 1) Housing – provides temporary housing for homeless patients who still need medical assistance post hospitalization or discharge from a skilled nursing facility. 2) Meals – provides three nutritious and healthy meals a day and some snacks. Meals will be prepared according to the dietary needs of the patient. If qualified, meals-on-wheels can be arranged. 3) Evaluation and assessment upon admission.
Our Recuperative Care Program provides residential services to homeless individuals with acute medical conditions that would benefit from a respite program that provides case management, personal care, medical care and access to long-term housing resources and placement.
The Contra Costa Philip Dorn Respite Center is a 26 bed stand-alone facility that is open 365 days per year, 24/7. Program participants have dorm or limited private accommodations, medical and psychiatric services and follow-up, meals, case management, benefits and housing assistance.
Gospel Center Rescue Mission (GCRM) Recuperative Care is the only non-profit program of its kind in San Joaquin County and a facility that provides 24-hour shelter beds for people who are too well to be in the hospital, but too sick to recuperate on the streets.
